Certificate Copy Request:

To protect the privacy of the individual involved, certificates are only issued to the person whose record is requested or a person who provides proof of parental authority, guardianship, power of attorney, executor/personal representative, or other legal status on behalf of the person whose record is requested. NO official certificates are issued for genealogical purposes. A photo ID is required for a copy of the certificate. We do ask for a $10 donation for a copy of the certificate. The certificate can take up to 2 weeks to process.
