Request for a Copy of Certificate:

To protect the privacy of the individual involved, certificates are issued only to the person whose registration is being requested or to someone who provides proof of parental authority, guardianship, power of attorney, executor/personal representative, or other legal status on behalf of the person whose registration is being requested. Official certificates are NOT issued for genealogical purposes.

• A photo ID is required to obtain a copy of the certificate.

• We request a donation of $10 for each copy of the certificate.

• The certificate may take up to 2 weeks to process.
